Counter-Notification (Restoration Request)
If you have received a notification regarding the removal of material under the DMCA, you may submit a counter-notification that includes:
Your physical or electronic signature
Identification of the removed material and the location where it previously appeared
A statement under penalty of perjury that you have a good-faith belief the material was removed due to a mistake or misidentification
Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the appropriate court as defined under 17 U.S.C. §512(g)(3)

Repeat Infringer Policy
